If the outcome was completed using the app, the clinical staff would be able to manually update the record on RIVIAM Web. 

If the outcome hasn’t been completed on the app, they can record the incident in the notes. In this case, when the outcomes are uploaded to RIVIAM, SystmOne/TPP would be updated with details of these issues.
