A professional referral via the website


A professional referrer can create a referral via the website. The user can launch the online form on the professional page by selecting 'Submit Referral'.

This online form can be built to the exact specifications required by the customer.

In this example, there are four sections to be completed on this online form.

  1. About you.
  2. Person referring.
  3. The person needing help and support.
  4. Support needed.

Some fields on the online form are mandatory based on customer requirements and must be completed to submit the form. When one page has been completed, select 'Continue' to move to the next page.

Once each page has been completed, select 'Submit Referral'.

A confirmation page will appear, displaying the referral number and contact details for the customer.

 

 Please Note: The form can be configured to allow professionals to select multiple options for the support needed and generate multiple referrals through a single form submission. Alternatively, the user can complete another referral form by selecting 'Start a new referral'.
